Why this matters
- 1 Stay reachable. Pastoral care, prayer chain calls, and time-sensitive announcements only work if we have your current phone and email.
- 2 Cover the whole household. Submit for spouse, children, and family living at the same address in one form — up to 5 people, one shared mailing address.
- 3 Keep it private. Your information stays inside the ALCOD Microsoft 365 tenant and is only accessible to the Member Services Ministry team. We never sell or share it.
What you'll need
- Title, legal name, and (if applicable) suffix
- Email and mobile — required for the head of household and any adult (18+) so we can reach you
- Mailing address (typed once, shared across the household)
- Date of birth (for birthday greetings and church records)
