- Salutation (Mr / Mrs / Ms / Dr / Rev / Pastor / etc.)
- Legal name — first, middle initial, last
- Email and mobile number
- Mailing address — street, city, state, zip
- Date of birth
All information stays inside the ALCOD Microsoft 365 tenant and is only accessible to the Member Services Ministry team.
How it's used
